Campaign to Support Community Hospice & Palliative Care Foundation
Community Hospice & Palliative Care Foundation is dedicated to advancing philanthropy to enable Community Hospice & Palliative Care to fulfill its mission and vision. Funds raised through the Foundation enhance quality of life for patients and families and support unfunded and under-funded patient care, outreach programs and services for those facing advanced illness and grief. Our donors enable us to continue to provide community programs and compassionate care to all who need it, regardless of ability to pay. There are a number of ways you can join our efforts to make a difference in the lives of area families living with advanced illness.

- Creating and developing clubs or activities for fundraising support through school
- Creating and hosting an event for the Community PedsCare patients and their families.
Since 2000, Community Hospice& Palliative Care has provided palliative and hospice care and support forchildren living with life-limiting and life-threatening conditions throughCommunity PedsCare, an award winning pediatric care program. The program, oneof only a few community pediatric programs in the United States offering bothpalliative and hospice care, provides clinical, social service, spiritual,child life, respite, medical and volunteer support to improve quality of lifefor children from prenatal to age 21. Our total family-centered care allows usto embrace and focus on the special needs of each child. To date, the programhas served more than 2,300 patients.
